To file a program discrimination complaint, a complainant should complete a Form AD 3027, USDA Program Discrimination Complaint Form (PDF, 293 KB), which can be obtained online, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA.The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation.

The Upper Darby High School Wall of Fame seeks nominations from a diverse range of achievements to provide examples of success for our current students. The first class was inducted in 1979 and since then over 70 alumni have been honored for distinguished academic achievements, professional accomplishments, service to the Upper Darby School District or community, musical and athletic honors as well as philanthropy. In the late 1970’s the Upper Darby High School Distributive Education Club (DECA) under the direction of Paul Motta began honoring distinguished alumni by establishing the Wall of Fame. Retired curriculum supervisor and social studies teacher, Liz McDonald, further developed the program into what it is today. Nominations are accepted through the website or by mail and reviewed by the committee for selection. The selection committee meets every two (2) years and the ceremony typically occurs in November. Past honorees as well as inductees and their guests are welcomed back to UDHS for an assembly with current students. The Wall of Fame is located on the walls of the hallway adjacent to the Board Room.

The Upper Darby School District's Application for Use of Facilities changed from the paper-based process to the on-line process through Facilitron.
There are four simple steps to making a request:

Step 2:
Register for an account at facilitron.com. (You only need to do this once.)
Step 3:
Login and make your request. You can view all space inventory, space attributes, and pricing of each space in the website.
Step 4:
Upon final approval by the School Board, bring the reservation confirmation page to your event.
